Information you choose to provide
The coverage-review and seminar-request forms ask for basic contact information and a general reason for your request. Do not submit Social Security or Medicare numbers, policy or account numbers, diagnoses, medication lists, medical records, payment information, or other highly sensitive information.
How the information is used
Two Waters uses the information to review and respond to your coverage or seminar request. A request does not obligate you to purchase insurance, enroll in a plan, or attend an event.
Where the request goes
Online requests are sent to a same-site Cloudflare form handler and stored in the private Cloudflare database used for this website. The site reports success only after the request is saved. A plain-text alert containing the information you submitted is then sent to the Two Waters business email inbox so the request can be reviewed. Records are kept only as long as reasonably needed for the request, an active conversation, or a legal, regulatory, contractual, or recordkeeping reason.
Website security checks
The forms currently use basic validation and automated-spam checks without asking you to complete a CAPTCHA. Cloudflare may process ordinary technical request information to deliver and protect the website. Additional spam protection may be added if it becomes necessary.
Sharing and selling
Two Waters does not sell contact information. Information may be shared only when reasonably necessary to respond to your request, provide a service you requested, operate the website, meet legal or regulatory obligations, or protect against fraud and misuse.
Your choices
You may ask about information you previously submitted or request that an unnecessary contact record be corrected or removed, subject to any legal, regulatory, contractual, or recordkeeping requirements that apply.
