The coverage review form asks for your first name and phone number, with optional fields for email address, the type of help you are seeking, and a short message. Please do not submit Social Security numbers, policy account numbers, detailed medical records, payment information, or other highly sensitive information through the website form.
How the information is used
The information is used to review your request, contact you, understand the type of insurance guidance you may need, and maintain an appropriate record of the conversation. Submitting the form does not obligate you to purchase insurance.
Where the request goes
Form submissions are stored in Wix Forms and Wix Contacts. An email notification is sent to the Two Waters site owner so Michael can respond. Website and form infrastructure is therefore also subject to Wix’s applicable privacy and security practices.
Sharing and selling
Two Waters does not sell the contact information you provide through this form. Information may be shared when reasonably necessary to respond to your request, provide requested insurance services, operate the website, meet legal or regulatory obligations, or protect against fraud and misuse.
Your choices
You may ask about information you previously submitted or request that an unnecessary contact record be corrected or removed, subject to any legal, regulatory, contractual, or recordkeeping requirements that apply.
